Do You Need A DOT Physical Medical Exam In Boise Idaho?

Our office is conveniently located in the middle of the Treasure Valley in Boise. We are located on the northwest corner of Maple Grove and Fairview, right next to Jack In The Box restaurant.

DOT stands for Department Of Transportation. You will need to get a "DOT" exam from a licensed medical examiner. The Wellness Center Of Boise is a licensed medical examiner.

This is considered an occupational health exam. If we determine you are physically qualified to drive commercial motor vehicles in Idaho we will furnish a copy of your results as well as complete a medical examination report.
This is what you should expect during your examination:
1. You will be greeted by our friendly office staff and given the medical examination form. (supplied by us)
2. General Information: You will fill out your name, SS#, birth date, age, sex, gender. If it’s a new certification, re-certification, or a follow-up. We will need address, telephone, driver license number and state of issue.
3. Health History: you will answer a yes or no to several questions ranging from current medications that you are taking, previous surgeries and any current health symptoms.
4. Physical Examination: Dr. Collin will check your:
  • VISION: You must have at least 20/40 acuity in each eye with or without correction (eyeglasses or contacts) and at least 70 degrees of peripheral vision on the horizontal meridian in each eye.You must also be able to recognize and distinguish standard traffic signal colors: red, green, and amber.,
  • HEARING: You must perceive a forced whisper voice at 5 feet or more with or without hearing aid. Otherwise, you must undergo a formal hearing test with an audiometer and pass. Dr. Foster musct be able to see your ear drum. If your wax is too thick and blocks your eardrum it is an automatic disqualification. Please irrigate your ears before your visit.
  • BLOOD PRESSURE: Your blood pressure must be less than or equal to 140/90 to be qualified as a driver. If your blood pressure is more than 140/90 but less than 180/110, you may qualify for a shorter Medical Examination Certificate. This depends on several things, including if this is an initial or re-certification and also on the discretion of Dr. Foster.
  • URANALYSIS: You will be asked to (go in a cup) so make sure you drink plenty of water prior to your exam. Your urine will be evaluated for specific gravity, sugar, blood and protein. The exam consists of an examination of mouth and

Starting May 21, 2014 all physicians have to be certified by the FMCSA. If your Doctor is not certified, your test will be voided.

Dr. Collin is a certified medical examiner by the national registry of motor carriers.
